In Idaho, we conduct functional assessments and recommend eligibility for Medicaid waiver services for adults and children.

DD Assessments with Liberty 

  • Scheduling: One of our Customer Support Specialists (CSS) will contact you to schedule your assessment. Please note that adult participants must be present for all assessments. Child participants must be present for full assessments, however, their presence is preferred for all assessments. Liberty may send SMS/MMS text appointment reminders for the applicant contacts on record.
  • Interview: The assigned assessor will ask questions to get to know your medical, social, and developmental history. The assessor will interview you and the person you choose to bring with you to learn about you and your needs. The assessor will conduct the assessment tool with a person of your choice who knows you very well. For adults, the assessor will conduct a needs inventory that will help to calculate your annual budget if you qualify for DD waiver services.
  • Documentation: The assigned assessor may request that you provide supporting documentation including medication lists, seizure logs, current health and physical records, incident reports, etc…

YES Assessments with Liberty 

  • Scheduling: One of our Customer Support Specialists (CSS) will contact you to schedule your assessment. Please note that your child must be present at the time of the assessment. There are no exceptions to this. Liberty may send SMS/MMS text appointment reminders for the applicant contacts on record.
  • Interview: The assigned assessor will be asking questions to get a lifetime history of symptoms, etc… for the initial assessment. Each annual assessment after that, will only be asking the questions to update for the last year. Please let your assessor know if there is something that is too uncomfortable or triggering for the child to be asked. Remember, you are the expert of your child!
  • Documentation: If you have a psychological assessment for your child, please have that information for your assessor. You will need a current list of medications, including dosage as well as the names of any treatment provider(s).

State of Idaho Eligibility Independant Assessment Opportunities

We offer eligibility assessments for the State of Idaho Development Disabilities (DD) services and children’s mental health services through Youth Empowerment Services (YES).
