Developmental Disability Waiver Assessments

The Adult and Children’s Developmental Disabilities Program provides a system of care that has a wide array of home and community-based services including intervention and support services.

Developmental Disability (DD) means a chronic disability of a person which appears before 22 years of age and:

  • Is attributable to an impairment, such as an intellectual disability, cerebral palsy, epilepsy, autism or other condition found to be closely related to or similar to one of these impairments that requires similar treatment or services.
  • Results in substantial functional limitations in three or more of the following areas of major life activity; selfcare, receptive and expressive language, learning, mobility, selfdirection, capacity for independent living, or economic selfsufficiency.
  • Reflects the needs for a combination and sequence of special, interdisciplinary or generic care, treatment or other services which are of lifelong or extended duration and individually planned and coordinated.

How to Apply?

There are three steps to apply for Developmental Disability (DD) waiver services:

  1. Apply for Medicaid
  2. Complete and return either the adult or child developmental disabilities application
  3. Complete an assessment with Liberty Healthcare

What about Katie Beckett?

Katie Beckett is a Medicaid program for children living at home with long-term disabilities or complex medical needs, who may be eligible for Medicaid services even if their family income is above Medicaid federal poverty guidelines. There are three steps to apply:

  1. Apply for Medicaid
  2. Complete the application packet sent to you by Liberty Healthcare. The packet will be sent to you after you apply for Medicaid.
  3. Participate in a Level of Care Determination Assessment with Liberty Healthcare

Next Steps

Once your completed application has been received by Liberty Healthcare, you will be contacted to schedule your assessment appointment. Please note that Liberty needs to be able to get ahold of you by phone to schedule your appointment.

What documentation will I need?

Please be prepared to provide documentation of developmental disability diagnosis and current health and physical. You may be asked to provide seizure logs, other medical documentation, and incident reports etc.. You will also need a current list of medications, including dosage as well as the names of any treatment provider(s).
